Event Waiver, Media Release & Photography Consent
Overview & Purpose
This document serves as the official Event Waiver, Liability Release, and Media Release for GUTZY Sunday Funday — PLAY, hosted by GUTZY Movement LLC, doing business as The GUTZY Movement™ (“The Company”).
Section 1 — Liability Release & Assumption of Risk
By purchasing a ticket to GUTZY Sunday Funday — PLAY (the “Event”), I, the undersigned attendee, acknowledge and agree to the following:
1.1 Voluntary Participation
I am voluntarily participating in the Event hosted by GUTZY Movement LLC, DBA The GUTZY Movement™, on Sunday, July 12, 2026, at a licensed vendor location to be disclosed only to registered attendees. I understand that the Event includes physical activities including but not limited to lawn games, yard games, movement through a shared venue space, and social interaction.
1.2 Assumption of Risk
I acknowledge that participation in the Event involves inherent risks, including the possibility of personal injury, accident, illness, or property damage. I voluntarily assume all risks associated with my participation in the Event, whether or not such risks are foreseeable at the time of this agreement.
1.3 Release of Liability
In consideration of being permitted to attend the Event, I hereby release, waive, discharge, and covenant not to sue GUTZY Movement LLC, DBA The GUTZY Movement™, its members, officers, employees, volunteers, agents, representatives, venue partners, and successors (collectively, the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me during or in connection with the Event, except in cases of gross negligence or willful misconduct on the part of the Released Parties.
1.4 Venue & Third-Party Liability
I acknowledge that the Event is held at a licesensed vendor location. I understand that GUTZY Movement LLC, DBA The GUTZY Movement™, does not control all conditions at the venue and is not responsible for injury or damage arising from venue infrastructure, conditions, or third-party services including but not limited to alcohol service.
1.5 Alcohol
I acknowledge that alcohol is available for purchase at the Event through a licensed vendor. I understand that GUTZY Movement LLC, DBA The GUTZY Movement™, is not responsible for the actions of attendees who have consumed alcohol, and that I am solely responsible for my own choices regarding alcohol consumption and my conduct thereafter. The licensed vendor is solely responsible for compliance with all state alcohol laws. The GUTZY Movement LLC does not provide, serve, or distribute alcohol and is not responsible for the vendor’s conduct or compliance.
1.6 Health & Medical Responsibility
I represent that I am in sufficient physical health to participate in the Event activities. I acknowledge that GUTZY Movement LLC, DBA The GUTZY Movement™, is not responsible for any medical conditions that may arise during the Event. I authorize emergency medical treatment at my expense if I am unable to consent.
Section 2 — Photography & Media Release
The GUTZY Sunday Funday — PLAY is a public community event. Photography and video recording will take place throughout the Event for purposes including but not limited to: marketing, social media, editorial content, promotional materials, and The GUTZY Movement™ brand documentation.
2.1 General Photography Consent
By attending the Event, I understand that photographs and video recordings may be taken in public areas of the venue throughout the duration of the Event. I grant GUTZY Movement LLC, DBA The GUTZY Movement™, and its authorized photographers and videographers, the irrevocable right and license to photograph, film, record, and use my image, likeness, and voice in connection with the Event for any lawful purpose, including marketing and promotional use, without compensation.
2.2 Opt-Out Option
Attendees who do not wish to have their image used in marketing materials may indicate this preference at check-in. An opt-out indicator (wristband or sticker) will be provided. Our photography team will make reasonable efforts to avoid capturing opted-out attendees in marketing content. Note: general crowd photography in a public venue cannot be fully controlled, and this opt-out applies specifically to intentional marketing and promotional use. Attendees who opt out of marketing photography should understand that they may still appear in general crowd photography or in photos shared by other attendees on personal social media. This opt-out applies only to The GUTZY Movement™’s official marketing use of their image.
2.3 Notecard Station Privacy
The Event includes a private notecard station where attendees may write personal reflections in sealed, private notecards. GUTZY Movement LLC, DBA The GUTZY Movement™, expressly prohibits photography and video recording at the notecard station. No images or recordings of attendees writing at the notecard station will be captured or used by The GUTZY Movement™ or its authorized photographers. Attendees are also asked to respect the privacy of other women at this station and refrain from personal photography in that area.
2.4 Personal Photography by Attendees
Attendees are welcome to take personal photographs throughout the Event for personal use. Attendees are asked to be mindful of other women’s privacy, particularly at the notecard station, and to obtain consent before posting photographs of other identifiable individuals to public social media.
Section 3 — Notecard Station & Personal Data
3.1 Notecard Delivery
The Event includes an optional notecard station where attendees may write a private note to themselves. Attendees may choose to receive their notecard by: (a) providing a mailing address for postal delivery, or (b) providing an email address for digital delivery via scanned image. Mailing addresses and email addresses collected at the notecard station will be used solely for the purpose of delivering the notecard and will not be shared with third parties.
3.2 Birthday Month Collection
Attendees may optionally provide their birth month (month only — no date or year required) at the notecard station for the purpose of receiving a personal acknowledgment from The GUTZY Movement™ during their birthday month. This information is voluntary and will not be shared with third parties.
3.3 Email List
Attendees who provide an email address at the Event may be added to The GUTZY Movement™ email list to receive updates about programs, events, and offerings. Attendees may unsubscribe at any time. Email addresses will not be sold or shared with third parties.
